Conduct call with CBM-HC to decide which Care Sequence® plans are best for your program

Decide and share with CBM-HC who the contact at your program is to receive updated Care Sequences®. Updates are made based on guideline changes and based on continuous improvement of the 4R Oncology® model. This person will also get a 5-minute survey every 3-4 months with basic questions about how use of Care Sequences® is going.

Update the contact sheet(s) aligned with the Care Sequences® your program plans to use, or use “blank” version of contact sheet

Get Care Sequence® plans printed, recommend printing in color, on cardstock (65-100 lb), 2 sided with Care Sequence® plan on one side and the contact sheet on the reverse side.

Determine how Care Sequences® will be shared with people with cancer served by your program.

Determine and distribute Care Sequences® in your clinic, common methods:

  • Patient folder, if used
  • Copies of Care Sequences® in each clinic room
  • Copies of Care Sequences® in clinician work areas
  • Electronic copies of Care Sequence® forms stored on shared network server

Finalize the documentation approach for Care Sequence® plans:

  • Take a picture of the Care Sequence® plan using EMR technology (e.g. Haiku for EPIC)
  • Scan a copy and have patient leave with original
  • Make a photo copy and have patient leave with the original
  • Paste picture of Care Sequence® into medical record documentation
