  • Review current patient-facing materials, update contact and resource templates
  • Determine which Care Sequence templates you will use
    • Review Care Sequence® plan templates and determine which to start with
    • Determine if care delivery optimizations are needed to deliver care according to Care Sequences®

E. Prepare for implementation and rollout

  • Create a roll-out plan, starting with one clinician, one clinic; then rolling out more broadly
  • Get Care Sequence® templates printed.
  • Select a method to document Care Sequence® in Electronic Medical Record
