- Review current patient-facing materials, update contact and resource templates
- Determine which Care Sequence templates you will use
- Review Care Sequence® plan templates and determine which to start with
- Determine if care delivery optimizations are needed to deliver care according to Care Sequences®
E. Prepare for implementation and rollout
- Create a roll-out plan, starting with one clinician, one clinic; then rolling out more broadly
- Get Care Sequence® templates printed.
- Select a method to document Care Sequence® in Electronic Medical Record
