Colorectal Cancer Care Sequences®

 

4R Care Sequences® are effective, proven guideline-based care plans for providers and patients at diagnosis and at transitions between treatments.

Care Sequence® plans are part of the 4R Oncology® model, a set of proven tools for patients and their care teams to support personalized care planning and delivery.

These tools incorporate national guidelines for direct cancer care, supportive care, health maintenance, and social care into actionable plans.

 

 

A Care Sequence® provides an outline and approximate timeline of a patient’s care over several months, in the order this care should occur.

You may use different Care Sequences during each part of a patient’s care. Each rectangle on the Care Planning Map is a separate Care Sequence plan.

The Care Sequence® is a “care roadmap”.  It depicts timing & sequence of time sensitive care and integrates multidisciplinary cancer, supportive and social care.
